GENERALSUMMARY/ OVERVIEW STATEMENT:

Medication reconciliation during transitions of care from the community to hospitalsetting is essential for patient safety and to optimize inpatient care. This processbegins with a thorough history of all regular medication use prior to admission(prescribed and non-prescribed), using several different information sources.The medication history pharmacy technician will systematically interview thepatient, family, and/or caregiver to obtain a complete list of home medications. This medication list will then be confirmed with a secondsource that may include a pharmacy, mail order, or nursing facilitydocumentation. Following completion, the pharmacy technician will documentfindings in the medical record and communicate pertinent medication informationwith the medical team.

PRINCIPALDUTIES AND RESPONSIBILITIES:

InformationGathering
  • Collaborate with the healthcare team to identify patientsrequiring a medication history
  • Determine an appropriate time to conduct patient/familymember interviews
  • Interact with patient and family members in a professionaland compassionate way
  • Contact pharmacies, long-term care facilities, and clinicsto obtain additional information and confirm information provided by thepatient
  • Obtain medication allergy history and related reactions tothe medication
  • Obtain immunization history

  • Verificationof patient and medication information
  • Assesses patient compliance and knowledge about eachmedication through use of open-ended questions and motivational interviewingskills
  • Use good clinical judgement to assess accuracy of eachmedication by reviewing medication dose, route, frequency, and indication foruse
  • Review appropriate information with members of the medicalteam and pharmacist if applicable

  • Documentation
  • Navigate EPIC with proficiency
  • Document gathered information appropriately in EPIC andRedCap using approved templates



  • Qualifications


    QUALIFICATIONS:
  • Certified Pharmacy Technician (CPhT) and MA Pharmacy Technician license required
  • Nationally certified PTCB
  • Experience interacting and communicating with patients in the hospital or clinic setting
  • Strong foundational knowledge of generic and brand medications used to treat chronic conditions
  • Proficiency in communicating medication information with healthcare professionals

  • SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
  • Speak, read, and document the English language
  • Dependable with effective interpersonal skills
  • Ability to prioritize tasks and work independently
  • Precise and accurate in information gathering and documentation

  • WORKING CONDITIONS:
  • Workspace located on inpatient floor or emergency department
  • Directly communicating with patients/families/caregivers alongside the multidisciplinary medical team
  • Ability to stand or sit for long periods of the work day
