Reporting to the Rural Primary Health Care Manager, the Primary Health Care Nurse is an integral member of the primary health care team, providing and coordinating initial, continuing and comprehensive nursing services to patients in the PHC sites. The Primary Health Care Nurse promotes quality health care outcomes working within established standards for case management practice. The Primary Health Care Nurse initiates and/or participates in community development initiatives and participates in the evaluation of Primary Health Care initiatives within the site.


Education: Graduate from an approved School of Nursing. Baccalaureate Degree in Nursing; Current registration with the Saskatchewan Registered Nurses Association (SRNA); Basic Life Support Certification. Experience: Minimum two (2) years of recent nursing experience in a community based setting is required. Previous experience in teaching, facilitation skills and leading group activities is required. Experience in case management is an asset. Knowledge, Skills and Abilities: Demonstrated knowledge of chronic disease prevention and management. Demonstrated clinical competency including critical thinking skills and confidence to work autonomously. Demonstrates sound evidence based knowledge and judgment when making decisions about a client's nursing care. Demonstrates knowledge of population health promotion theory and practice and ability to transfer this knowledge into practice. Knowledge of and sensitivity to the impact of social, economic , environmental and cultural issues on chronic disease. Team leadership and facilitation skills including an understanding of the fundamentals of team dynamics. Demonstrates the ability to manage conflict toward effective outcomes. Effective communicator with active listening and motivational interviewing skills. Strong computer skills. Able to prioritize tasks and work under pressure. Knowledge of the lean management systems. Additional Information: Must possess a valid driver's license. Salary subject to SUN/SAHO Position Evaluation. Scheduled hours of work may be reduced (if applicable) during the rotation stipulated when a statutory holiday is recognized during that period. Hours of Work: 170 HR/6 WK; 8 HR
